How low testosterone is actually confirmed

Testosterone naturally peaks in the morning, so a single afternoon draw tells us very little. We confirm low levels with two early morning total testosterone tests and pair them with free testosterone, LH, FSH, estradiol, hematocrit, PSA where appropriate, thyroid, and a metabolic panel.

That workup matters because the fatigue and low drive that sent you searching can also come from sleep apnea, poorly controlled blood sugar, thyroid dysfunction, or medication side effects. Treating a level without understanding the cause is how men end up on therapy they never needed.

What monitored therapy looks like here

Injections

Weekly or twice weekly dosing gives the tightest control and the fastest adjustments. Many men give them at home after we teach the technique.

Pellets

A short in office insertion every few months for men who would rather not think about a weekly schedule.

Scheduled rechecks

Labs at roughly six to eight weeks, then on an ongoing interval, watching testosterone, estradiol, hematocrit, and PSA.

Fertility conversation up front

Testosterone therapy can suppress sperm production. If children are in your plans, we discuss that before starting, not after.

Safety and honest expectations

The FDA requires clear labeling on testosterone products, and long term data has continued to refine what is known about cardiovascular risk. Our position is straightforward: therapy belongs to men with confirmed low levels and real symptoms, and it belongs under monitoring.

Energy, mood, and libido often shift first. Strength and body composition changes take longer and still depend on training, protein, and sleep. We do not promise numbers, and we will tell you when testosterone is not the answer.
