The symptoms women describe most

Sleep usually breaks first. Women tell us they fall asleep fine and wake at three, or wake soaked and cannot get back down. Then cycles shorten or become unpredictable, patience gets thin, workouts feel harder, and the words they want stop arriving on time.

Vaginal dryness, painful intimacy, and recurring urinary symptoms come up less often in conversation and matter just as much. We ask directly, because these are among the most treatable symptoms and the ones women most often assume they simply have to live with.

How we evaluate before treating

Perimenopause is diagnosed clinically. Hormone levels swing so widely month to month that a single FSH or estradiol value can be misleading, and we will tell you when a test would not change the plan.

What we do check reliably is thyroid function, iron and ferritin, vitamin D, a metabolic panel, and lipids, because thyroid disease and anemia mimic these symptoms closely. Skipping that step is how women spend a year on the wrong treatment.

Treatment options, including options that are not hormones

Systemic estrogen therapy

For hot flashes, night sweats, and sleep disruption, delivered as a patch, cream, or pellet, with progesterone when the uterus is present.

Low dose vaginal estrogen

Targeted treatment for dryness, discomfort, and urinary symptoms, with minimal systemic absorption.

Testosterone where appropriate

Considered for persistent low libido after other causes are addressed, at physiologic dosing for women.

Non hormonal support

Sleep, protein, strength training, alcohol timing, and non hormonal prescription options when hormones are not appropriate or not wanted.
