How the dose ladder works

Semaglutide is not started at a therapeutic dose. Treatment begins low and steps up over weeks, which is what keeps nausea manageable and lets your body adapt to slower gastric emptying.

Some patients respond well at a middle dose and never need the top of the ladder. Others need the full titration. We move at the speed your body allows and hold when side effects need time to settle, rather than following a calendar for its own sake.

Branded and compounded, explained without spin

FDA approved branded semaglutide has the strongest evidence behind it and clear labeling. Compounded semaglutide comes from a licensed pharmacy and is not FDA approved as a finished product, and the FDA has warned specifically about salt forms and about products sold outside legitimate pharmacy channels.

We tell you which category any medication we discuss falls into, where it comes from, and what the trade offs are on cost and oversight. What we will not do is let marketing language blur that line.

The first four weeks, week by week
